The Milestone System: Why Specific Days Matter

The milestones in the Relapsd protocol are not arbitrary. Each corresponds to a documented neurobiological event in the recovery arc.

The milestone system in the Relapsd app marks specific days: 1, 3, 7, 14, 30, 60, 90, 180, and 365. These are not arbitrary motivational checkpoints. Each corresponds to a documented neurobiological threshold.

Day 1: The Commitment

The 24-hour mark. Your system has cleared nicotine, THC metabolites are beginning to clear, or GABA-A/NMDA rebalancing has begun. The commitment to the protocol is made.

Biological significance: For nicotine-the last nicotine has been metabolized. For cannabis-THC serum levels dropping significantly. For alcohol-first 24 hours past the highest seizure risk window.

Day 3: The Peak

72 hours. The most commonly referenced milestone in cessation. For nicotine, this is the peak of nAChR withdrawal signaling. For cannabis, peak acute symptoms. For alcohol, the critical window for DTs has passed if it was going to occur.

Why this matters: Getting to day 3 is the first test of the protocol. The first 72 hours are the most acutely uncomfortable period for nicotine and cannabis cessation.

Day 7: One Week

Seven days. The acute phase of withdrawal is substantially complete for nicotine and largely resolving for cannabis. Alcohol GABA-A/NMDA balance has substantially normalized.

Biological significance: nAChR downregulation reversal is measurable. CB1 receptor upregulation has begun.

Day 14: The Second Peak

The dopamine deficit for nicotine peaks around day 14-21. Cannabis REM rebound is typically at its most intense in weeks 1-2. This milestone marks the beginning of the second challenge after the acute phase.

Day 30: One Month

One month of abstinence. nAChR density substantially normalized for most nicotine users. CB1 receptor density measurably recovering. Liver enzyme improvements visible for DRY users.

This is the first milestone where neuroimaging studies show measurable structural brain recovery. Gray matter volume recovery begins to be detectable.

Day 60: Two Months

End of the 8-week protocol period for NIC and GREEN. The receptor-level withdrawal is substantially resolved. What remains is primarily behavioral (conditioned cue responses).

Day 90: Three Months

The three-month mark is consistently identified in cessation research as a threshold after which sustained abstinence becomes substantially more stable. Relapse rates fall significantly after this point.

For cannabis specifically: PET imaging studies show CB1 receptor density approaching non-user levels at ~90 days for moderate users.

Day 180: Six Months

Six months. The social-context relapse risk has been significantly tested. Most high-risk social situations have been encountered and managed. The non-user identity has been exercised and strengthened.

Day 365: One Year

One year. This is the gold standard in cessation research-most studies define long-term abstinence as 12-month abstinence. Brain recovery studies show near-complete normalization at one year for most users.

Celebrate this milestone. It is clinically meaningful.
