Ashwagandha (Withania somnifera) appears in dozens of supplement products with wildly varying quality. The GREEN protocol specifies KSM-66 for reasons that matter clinically.
What Ashwagandha Does
Ashwagandha is an adaptogen-a compound that modulates the HPA (hypothalamic-pituitary-adrenal) axis, the body's primary stress response system. The primary active constituents are withanolides, a class of steroidal lactones with complex neuroendocrine effects.
The most clinically relevant effects:
- Cortisol reduction: Multiple RCTs have demonstrated significant cortisol reductions with ashwagandha supplementation, with effect sizes of 15-30% from baseline
- Anxiety reduction: Consistent anxiolytic effect in RCTs, with effect sizes comparable to low-dose pharmaceutical intervention in some trials
- GABA modulation: Some withanolides have demonstrated positive modulation of GABA-A receptors-relevant for the GABAergic component of cannabis withdrawal anxiety
- Thyroid support: Some evidence for thyroid hormone normalization, relevant for the metabolic dysregulation during cessation
Why Specifically KSM-66
KSM-66 is a proprietary full-spectrum ashwagandha root extract standardized to minimum 5% withanolides using only root material (no leaves). The evidence base for KSM-66 specifically includes:
- A 2012 RCT showing significant cortisol reduction and anxiety improvement
- A 2019 RCT showing significant improvements in stress, anxiety, and sleep quality at 240mg daily
- A 2021 double-blind RCT with 600mg showing reductions in anxiety, cortisol, and improvements in cognitive function
The key point: these trials used KSM-66 specifically, with consistent results. Generic ashwagandha extracts with lower or unspecified withanolide content, or leaf-based extracts, cannot be assumed to produce equivalent effects.
Why It Matters for Cannabis Cessation
Cannabis withdrawal produces cortisol dysregulation: the ECS's modulation of the HPA axis is impaired, leading to elevated cortisol responses to stress. This contributes to the anxiety, irritability, and sleep disruption of the withdrawal period.
Ashwagandha's cortisol-modulating and HPA-adaptive effects are specifically relevant to this mechanism. It does not address receptor-level dysfunction (that requires time and abstinence) but supports the physiological stress response system during the recovery period.
Dosing
Standard clinical dose: 600mg KSM-66 daily, with a meal. This is the dose used in most positive RCTs.
Timing: Morning dosing is typical, but evening has some evidence for sleep quality improvement. Given the dual benefit, some users split the dose (300mg morning, 300mg evening).
Duration: Full 8-week protocol. HPA axis normalization is a gradual process; the benefits accumulate over weeks rather than producing acute effects.
What to expect: Reduced intensity of stress responses rather than elimination of stress. The anxiolytic effect is meaningful but not sedating. Most users notice a reduction in the physiological anxiety response (heart rate, tension) more than a change in cognitive anxiety content.
