Thiamine and Wernicke's Encephalopathy: The Most Medically Critical Part of the DRY Protocol

Alcohol impairs thiamine absorption and depletes stores. The result, if severe, is irreversible neurological damage. This is the medical emergency hidden inside alcohol withdrawal.

The most urgent medical intervention in the DRY protocol is not kudzu or GABA supplementation. It is thiamine.

Thiamine (vitamin B1) deficiency in the context of alcohol use disorder is common, underrecognized, and capable of producing permanent neurological damage. This is not a hypothetical risk.

The Mechanism of Depletion

Alcohol impairs thiamine at multiple levels:

Intestinal absorption: Alcohol directly inhibits the thiamine transporter in the intestinal mucosa, reducing absorption efficiency by 50-70% in heavy drinkers.

Hepatic storage and phosphorylation: The liver converts dietary thiamine to its active form (thiamine pyrophosphate, TPP). Alcohol-induced liver disease impairs this conversion.

Increased excretion: Alcohol increases urinary thiamine excretion.

Dietary insufficiency: Heavy drinkers frequently have poor nutritional intake; thiamine-rich foods (legumes, whole grains, meat) are often underconsumed.

The cumulative result: even with nominal dietary thiamine intake, heavy alcohol users can develop significant depletion within weeks to months.

Wernicke's Encephalopathy

Thiamine is a required cofactor for multiple critical metabolic enzymes in the brain, including:

  • Pyruvate dehydrogenase (glucose metabolism)
  • Alpha-ketoglutarate dehydrogenase (Krebs cycle)
  • Transketolase (pentose phosphate pathway)

Without adequate thiamine, these pathways fail. In the brain, this produces localized energy failure in specific thiamine-sensitive regions: the medial thalamus, mammillary bodies, periaqueductal gray matter, and cerebellum.

The clinical triad of Wernicke's encephalopathy:

  1. Confusion/altered consciousness: Ranging from mild disorientation to stupor
  2. Oculomotor abnormalities: Nystagmus, ophthalmoplegia (eye movement paralysis)
  3. Ataxia: Loss of coordination, gait instability

The classic triad is present in only 16-33% of pathologically confirmed cases (autopsy studies). Many cases are subclinical or present atypically, and are therefore missed.

Without treatment, Wernicke's progresses to Korsakoff Syndrome in 80% of cases: a permanent amnestic disorder characterized by severe anterograde amnesia (inability to form new memories), confabulation, and executive dysfunction. Korsakoff Syndrome has no effective treatment and is irreversible.

With immediate IV thiamine administration (hospital setting), Wernicke's is partially reversible in many cases. Oculomotor findings resolve most reliably; cognitive and gait abnormalities may partially recover.

The DRY Protocol Response

Every DRY order includes thiamine 100mg. This is a medical decision, not a wellness supplement choice.

The oral dose of 100mg is appropriate for prevention and initial repletion in non-hospitalized individuals with moderate depletion. For suspected severe Wernicke's, intravenous thiamine (200-500mg, 3 times daily) is required-this cannot be achieved orally and requires emergency medical treatment.

The instruction: If you or anyone around you develops confusion, eye movement problems, or loss of coordination in the context of alcohol withdrawal, this is a medical emergency. Go to an emergency room. Request IV thiamine before any glucose administration (glucose without thiamine can precipitate Wernicke's in thiamine-depleted individuals).

B6 and B12

Alcohol also impairs B6 (pyridoxine) and B12 (cobalamin) absorption and metabolism. B6 is required for GABA synthesis (directly relevant to withdrawal); B12 deficiency contributes to peripheral neuropathy (common in long-term alcohol users) and cognitive impairment.

The B-complex in the DRY protocol addresses all three deficiencies. The thiamine dose (100mg) exceeds the standard B-complex dose because the clinical evidence supports high-dose thiamine specifically.

This is the most medically serious component of the DRY protocol. Take it seriously.
