Why 0.3mg Melatonin Is Better Than 5mg

Standard melatonin doses (5-10mg) suppress endogenous melatonin production. The physiologically appropriate dose is 0.3mg. Most products are 10-17x too high.

Melatonin is the most widely used sleep supplement in the United States. The standard commercial dose-5-10mg-is pharmacologically inappropriate for most uses. This matters during cessation because sleep architecture is already disrupted, and adding exogenous melatonin at the wrong dose can worsen rather than improve sleep quality.

The Physiology

The pineal gland produces melatonin in response to darkness, with serum levels rising in the evening and peaking in the early morning hours (1-3am). The physiological peak serum melatonin level is approximately 100-150 picograms/mL.

A standard 5mg melatonin dose produces serum levels of approximately 1000-10,000 picograms/mL-10 to 100 times the physiological peak.

The brain's melatonin receptors (MT1 and MT2) are exquisitely sensitive. At physiological concentrations (100-150 pg/mL), they mediate the normal sleep-entraining and sleep-promoting effects of melatonin. At pharmacological concentrations (1000+ pg/mL), they are saturated-the receptors receive the maximum signal regardless of whether you took 1mg or 20mg.

The Suppression Problem

Exogenous melatonin suppresses the pineal gland's own melatonin production through a negative feedback mechanism. High-dose melatonin taken regularly can produce a form of "melatonin dependence"-not in the addiction sense, but in the sense that the body reduces its own production, and sleep suffers when the supplement is not taken.

This is the opposite of what cessation users need: a sleep architecture that improves with sustained abstinence.

The Research on Low-Dose Melatonin

Dr. Richard Wurtman at MIT, who discovered melatonin's role in circadian rhythm regulation, conducted the seminal dose-response studies:

  • 0.3mg melatonin produces serum levels within the physiological range
  • 0.3mg improves sleep onset time comparably to 3-5mg
  • 0.3mg does not suppress endogenous production
  • Elderly individuals show greater sensitivity; the same dose effect occurs

A 2001 Cochrane-reviewed study confirmed that low-dose melatonin (0.1-0.3mg) is effective for sleep onset in a dose that matches physiological levels, without the adverse effects of higher doses.

Why Commercial Products Are 10-17x Too High

The answer is commercial, not scientific. In the US, melatonin is sold as a supplement, not a pharmaceutical. There is no FDA dose regulation for supplements. Manufacturers have incentive to produce higher doses because higher numbers are perceived as more effective and more premium.

Most Americans using 5-10mg melatonin nightly are taking pharmacological doses with no additional sleep benefit over 0.3mg, and with the risk of endogenous suppression.

The GREEN Protocol Dose

0.3mg melatonin, 30-45 minutes before desired sleep onset time.

Finding 0.3mg tablets is increasingly possible as awareness of the dose issue has grown. Alternatively, a 1.5mg tablet cut into quarters, or a 3mg tablet cut into tenths (with a pill cutter). Many liquid melatonin formulations allow 0.3mg doses.

Timing consistency: Melatonin's circadian-entraining effects depend on consistent timing relative to desired sleep time. Taking it at 10pm one night and 1am the next reduces efficacy.

The low-dose melatonin in the GREEN protocol is not window dressing. It is physiologically appropriate sleep support for a period of profound sleep disruption.
