"Drink water" is standard cessation advice, typically offered without mechanism and therefore dismissed as filler. It is not filler.
Dehydration Mimics Withdrawal
Many withdrawal symptoms are amplified or even produced by dehydration:
Headache: The most common withdrawal symptom is also the most common dehydration symptom. Differentiating them is impossible without addressing hydration first.
Fatigue: Both dehydration and withdrawal produce fatigue through different mechanisms. Dehydration reduces blood volume, reducing oxygen delivery to the brain. The overlap masks the contribution of each.
Difficulty concentrating: Cognitive impairment is documented with even mild dehydration (1-2% body weight fluid loss). Adding this to withdrawal-related cognitive impairment compounds the subjective experience significantly.
Irritability: Dehydration produces irritability through cortisol elevation and reduced cognitive resources for emotional regulation.
Nausea: Both alcohol and cannabis withdrawal produce nausea. Dehydration exacerbates it through multiple mechanisms including reduced gastric emptying.
Hydration and Cortisol
Dehydration activates the HPA axis (the same stress response system that drives substance craving). A 2-3% fluid deficit produces measurable cortisol elevation. This cortisol increase overlaps mechanistically with the stress-induced relapse pathway.
Maintaining adequate hydration keeps HPA activation from the hydration-depletion pathway at baseline, preserving HPA resources for managing actual stressors.
Specific Contexts Where Hydration Matters Most
Morning: The overnight fast produces mild dehydration by waking. Drinking 500ml (16oz) of water before coffee or food reduces the morning cortisol surge slightly and provides a clean behavioral anchor for the morning protocol.
During exercise: Cessation coincides with a period of increased exercise. Sweat losses need replacement. Dehydration during exercise reduces the BDNF response to exercise (the primary cognitive benefit).
During nicotine craving: Cold water specifically-the act of drinking a large glass of cold water produces physiological arousal (mild cold shock response) that briefly interrupts the craving's phenomenological grip, and the temperature change creates an oral sensation that partially addresses the oral habit.
For DRY protocol users: Alcohol is a diuretic; daily heavy drinkers are typically chronically dehydrated. The first week of cessation, before adequate intake is established, continues this deficit. Aggressive hydration (3+ liters/day) in week 1 supports overall physical recovery.
How Much
The evidence-based minimum: 2.5 liters (85oz) for men, 2 liters (67oz) for women from all fluids daily. Coffee counts toward this, despite being mildly diuretic-the mild diuresis is outweighed by the fluid volume.
During active cessation, target the upper end of this range. Physical symptoms of cessation typically reduce fluid intake (nausea, reduced appetite for food and drink). Deliberately compensate.
The cold water protocol: one large glass (250-500ml) of cold water when a craving begins. This is a simple, free, always-available craving interrupt.
