Blood Sugar Stability and Craving: The Glucose Connection

The relationship between blood glucose and substance craving is not commonly discussed in cessation literature. It is practically significant.

The Mechanism

Blood glucose drops trigger the release of two hormones relevant to cessation:

Cortisol: Hypoglycemia activates the HPA axis to raise blood glucose. Cortisol, as described in the cortisol-relapse article, directly activates the mesolimbic dopamine system and promotes substance-seeking behavior.

Orexin (hypocretin): Glucose-sensing neurons in the hypothalamus activate orexin neurons in response to low glucose. Orexin directly activates the VTA dopamine neurons of the mesolimbic pathway and is increasingly recognized as a key mediator of substance craving.

The practical implication: a blood glucose drop produces a cortisol/orexin-mediated activation of the craving circuit-independent of any external substance cue.

Why Nicotine Users Are Particularly Vulnerable

Nicotine stabilizes blood glucose through multiple mechanisms, including direct effects on insulin signaling and appetite suppression that reduces carbohydrate-heavy snacking. When nicotine is removed:

  • The blood glucose-stabilizing effect of nicotine is lost
  • Appetite rebounds, leading to episodes of excessive eating followed by drops
  • Blood glucose variability increases relative to the nicotine-maintained state

Dietary Blood Sugar Management

The most effective dietary interventions for blood glucose stability during cessation:

Eat regular meals: Three meals with protein and fat, at consistent times. Skipping meals produces the blood glucose drops that trigger the cortisol/orexin craving pathway.

Protein + fat at each meal: Protein and fat slow gastric emptying and reduce post-meal glucose spikes. A pure carbohydrate meal produces a spike followed by a drop; the same carbohydrates with protein and fat produce a flatter, more sustained glucose curve.

Reduce refined carbohydrates: White bread, sugar, sweetened drinks produce rapid glucose spikes followed by compensatory drops. These drops are direct craving triggers.

Don't skip breakfast: The cortisol awakening response (normal morning cortisol spike) combined with overnight fasting creates the highest glucose variability window of the day. Breakfast-particularly with protein-stabilizes this.

Low-glycemic carbohydrates: Oats, legumes, sweet potatoes, and most vegetables provide sustained energy without the glucose spike/crash cycle.

The Craving Snacking Substitution

During cessation, especially NIC, the oral habit drives snacking. The blood glucose management strategy: if snacking is inevitable, snack on foods that don't produce glucose spikes:

  • Nuts (fat and protein; minimal glucose effect)
  • Vegetables with hummus (fiber and protein)
  • Greek yogurt (protein; low glycemic)
  • Cheese (fat and protein; zero glucose effect)

These satisfy the oral habit without contributing to the glucose variability that amplifies craving.
