N-Acetylcysteine (NAC) appears in all three Relapsd protocols-NIC, GREEN, and DRY. That is not an accident. NAC is not a substance-specific tool. It is a glutamate homeostasis agent, and glutamate dysregulation is a common mechanism underlying all three forms of dependence.
This article focuses on how NAC addresses nicotine-specific glutamate disruption.
Glutamate and Nicotine Addiction
Dopamine gets most of the attention in addiction neuroscience-and for good reason, it is the reward signal. But the glutamate system is the learning and memory arm of the addiction circuit. Glutamate, the brain's primary excitatory neurotransmitter, encodes craving memories and cue-response associations.
Here is the problem: chronic nicotine use disrupts glutamate signaling in the nucleus accumbens and prefrontal cortex. Specifically, it reduces the activity of the cystine-glutamate antiporter (xCT), a transporter responsible for maintaining extracellular glutamate at stable, low levels.
When xCT function is reduced:
- Extracellular glutamate levels drop
- The brain's tonic (background) glutamate signal decreases
- This change in basal glutamate tone makes cue-triggered glutamate surges feel more dramatic and more compelling
- The result: environmental cues (a particular time of day, a cup of coffee, stress) trigger intense, hard-to-ignore cravings
This is the neurological mechanism behind the phenomenon of "trigger cravings"-the experience where the urge to smoke is not constant but is suddenly overwhelming in response to a specific context.
How NAC Works
NAC is a precursor to cysteine, which the xCT antiporter uses to exchange for extracellular glutamate. When NAC availability increases:
- xCT activity increases
- Extracellular glutamate levels normalize
- Basal glutamate tone is restored
- Cue-triggered glutamate surges are less exaggerated
- Cue-triggered cravings are subjectively weaker
The clinical translation: NAC does not eliminate cravings. It reduces their intensity, particularly the cravings triggered by environmental cues. This is a specific, documented mechanism-not a supplement industry claim.
The Clinical Evidence
Randomized controlled trials directly supporting NAC for nicotine cessation are limited. The strongest evidence base is for cocaine and cannabis use disorders, where multiple RCTs show significant reductions in craving and use. The proposed mechanism (xCT/glutamate restoration) is the same across substances.
For nicotine specifically, a 2013 trial by Prado et al. found that NAC supplementation significantly reduced cigarette cravings and consumption in smokers, with particular effect on cue-triggered cravings. The effect size was modest, but the mechanism was confirmed.
Given the strong mechanistic basis and favorable safety profile, the inclusion of NAC in a multi-modal nicotine cessation stack is well-supported even without large Phase III trials.
Dosing and Timing
Dose: 600mg twice daily. This is the dose used in most clinical trials. Higher doses (1200mg twice daily) are sometimes used in clinical settings but may cause gastrointestinal side effects without proportional benefit.
Timing: With food. NAC on an empty stomach can cause nausea. Morning and early afternoon dosing is typically well-tolerated.
Onset: NAC begins producing measurable effects on glutamate tone within 1-2 weeks of consistent use. Do not expect immediate craving reduction; allow 10-14 days before assessing efficacy.
Duration: Continue through the full 8-week protocol. Glutamate normalization takes time to establish, and the most important cue-response rewiring occurs in the first 8 weeks of abstinence.
The Other Benefit: NAC as Antioxidant
Nicotine combustion products (if smoking vs. vaping) produce significant oxidative stress. Even vaping generates some oxidative burden. NAC's role as a glutathione precursor makes it a reasonable antioxidant support regardless of cessation context.
This is a secondary benefit, but relevant for heavy smokers transitioning off cigarettes.
The Combination: NAC + Cytisine
NAC and cytisine address different aspects of nicotine dependence:
- Cytisine: receptor pharmacology (reduces craving at the nAChR level)
- NAC: glutamate homeostasis (reduces the intensity of environmental cue-triggered cravings)
Together, they form a mechanistically complementary pairing. This is the logic behind combining them in the NIC protocol.
